Cheekbone Augmentation Before & After | Cheek Filler for Midface Structure

The goal was midface restoration without a deep-plane lift. Cheekbone definition had softened over a decade and the lower face had started its early descent.

Midface assessment showed bilateral flattening of the malar prominence with mild associated descent of the cheek soft tissue and early nasolabial fold deepening. The bone structure underlying the soft tissue was well-formed. The pattern reflected age-related midface volume loss rather than excessive skin laxity. The plan was structural cheek filler placed at the deep periosteal level with the volume calibrated to project the malar prominence forward and provide indirect support to the lower face.

A firmer hyaluronic acid filler was placed via cannula at the deep malar position bilaterally. Deep periosteal placement gives bone-like support to the midface soft tissue rather than diffusing through the surface layer. The lower-face lift effect comes from re-establishing the midface anchor that drops with age. In the after image, the cheekbone projection is restored, the midface reads as supported, and the early lower-face descent has improved without direct lower-face treatment. Restoration over augmentation is the framework — the goal is the cheekbone the patient had a decade earlier, not a different cheekbone shape.
