How Body Filler Works
Two kinds of injectable do the work here, and they behave differently. Hyaluronic acid filler holds water and adds volume the day it is placed. Sculptra and Radiesse are biostimulators: they prompt the skin to lay down its own collagen, so the change builds over weeks rather than appearing at once.
Sculptra is poly-L-lactic acid. The particles sit in the deep dermis and the body treats them as a scaffold, forming collagen around them as they break down. The fullness is the new collagen, not the product, which is why it arrives slowly and holds.
Radiesse is calcium hydroxylapatite suspended in a gel. The gel gives an immediate lift, and the calcium microspheres stimulate collagen and elastin as the gel resorbs. It is thicker than a hyaluronic acid gel, so it suits the back of the hands and the crepey skin of the arms and thighs where structure is thin.
Why this cannot be undone the way lip filler can
A hyaluronic acid filler can be dissolved with an enzyme if the result is wrong or migrates. Calcium hydroxylapatite and poly-L-lactic acid cannot. There is no agent that breaks them down on demand; they resorb on their own schedule.
That is the reason volume is placed conservatively and built across more than one session rather than chased in a single visit. The safeguard against overfilling a body area is the first session being smaller than the eye wants.

Where filler is not the answer
Filler adds volume; it does not remove it. It will not take down a pocket of fat, and it does not cut or release the fibrous bands that tether a cellulite dimple, so deep dimpling needs a different procedure. A large increase in buttock size is the territory of surgery, not injectable shaping. Loose skin that must be removed is a surgical body lift or thigh lift, which Spectrum does not perform.

Every injection carries risk, and biostimulators carry a few that hyaluronic acid filler does not. These are the ones worth understanding before you decide.
Swelling, bruising, tenderness
Expected for a few days after placement, more so in larger areas and on the hands. A cannula lowers bruising but does not remove it.
Nodules or lumps
Biostimulators can form small firm nodules if the product clumps or is placed too superficially. Even placement and, for Sculptra, massaging the area as directed lower the chance.
It cannot be dissolved
Calcium hydroxylapatite and poly-L-lactic acid resorb on their own schedule and cannot be reversed with an enzyme the way a hyaluronic acid filler can. A conservative first session is the safeguard.
Vascular injection
Injecting into a vessel is the serious risk of any filler. Blunt cannula technique and knowing the planes of each area are how it is avoided.
You go over the risks, the off-label use, and the limits of what filler can change before anything is placed. Consent is part of the appointment, not a form handed over at the end.
How safety is handled here
Sculptra and Radiesse are approved for the face and the back of the hands. Using them to shape the hips, buttocks, or thighs is off-label, which is a statement about where the labelling stops, not a statement that the product does not work there. Dr. Munib uses them this way deliberately and explains the off-label status at the consultation.
Buttock injection in particular has anatomy that rewards caution, so volume is placed in planes that avoid the large vessels and built across sessions rather than forced in one.
