Smoker’s Lines (Vertical Lip Lines) Treatment in Irvine, CA
Smoker’s lines, the fine vertical lines above the upper lip, sometimes called barcode lines or perioral wrinkles, are one of the more stubborn lower-face concerns, and the name is misleading: most patients we treat have never smoked. They can come from repeated lip movement, collagen loss with age, sun exposure, and thinning skin, often in combination.
At Spectrum Skin Clinic in Irvine, Sabeen Munib, MD treats vertical lip lines with a tailored mix of Botox, hyaluronic acid filler, and resurfacing, but only after working out what is actually driving them in your case. Lines that appear only when you purse your lips are a different problem than lines etched in at rest, and what helps depends on which pattern you have. Dr. Munib performs every treatment personally.

Is This You?
Three common patterns Dr. Munib evaluates at consultation for vertical lip lines. Most patients recognize themselves in more than one.
Lines That Show Only When You Purse or Apply Lipstick
Your upper lip looks smooth at rest, but vertical lines appear when you pucker, sip through a straw, or apply lipstick, and lipstick has started to travel into them. This pattern is usually driven by muscle movement rather than etched-in skin change, and it often responds to a conservative approach.
Fine Lines Etched In Even When Your Face Is Relaxed
You can see the vertical lines in the mirror without moving your mouth at all. Static lines like these reflect collagen loss, thinning skin, and sun exposure over time, and they typically need a different strategy than movement-driven lines, sometimes more than one treatment type.
Tried Retinoids and Topicals Without Real Change
You have been diligent with retinol, peptides, or lip treatments and the surface looks a little better, but the vertical lines are still there. Topicals can support skin quality; they generally cannot fill an etched line or relax the muscle creating it, which is where in-office options come in.
Why Patients Choose Spectrum for Lip Lines
The area around the mouth is unforgiving. It moves constantly, the skin is thin, and over-treating it is one of the faster ways to make a face look unnatural. Patients come to Spectrum because Sabeen Munib, MD treats this region conservatively and personally. There is no injector you have never met, and no default protocol applied to every lip.
Because vertical lip lines usually have more than one cause, the goal at consultation is diagnosis before treatment: deciding whether movement, lost volume, surface texture, or some mix is the real driver, and being honest when a single treatment will only get you part of the way.

What’s Driving Your Lip Lines, and What Helps
| What you’re seeing | Likely driver | What Dr. Munib weighs | Typical direction |
|---|---|---|---|
| Lines appear only when you purse your lips or drink from a straw | Movement of the muscle that circles the mouth | Whether softening movement would blunt your natural expression | A conservative Botox micro-dose, placed shallowly |
| Fine vertical lines visible at rest, not just on movement | Etched-in static lines from collagen loss and sun exposure | Line depth, skin thinness, and bruising risk near the lip | A small amount of soft HA filler, at times with resurfacing |
| Lines plus a thinning or deflating upper lip | Volume loss along the lip body and border | How much support the lip can take before it looks overdone | HA filler to restore border support, lines addressed secondarily |
| Lines with crepey, sun-damaged texture across the upper lip | Surface photoaging, not the lines alone | Skin type, downtime tolerance, and pigment risk | Resurfacing such as fractional CO2 in selected patients |
How We Treat Smoker’s Lines, and Why It’s Often a Combination
There is no single best treatment for vertical lip lines; the right approach depends on what is creating them. In practice Dr. Munib draws on three tools, sometimes alone and often together.
Micro-dosed Botox or Dysport can relax the muscle that pleats the skin when you move your mouth. Around the lips this is deliberately light, enough to soften the pursing motion without affecting how you speak, eat, or smile. It does little for a line that is already etched in at rest.
Soft hyaluronic acid filler can support and smooth deeper static lines and, where the upper lip has deflated, restore a little border definition. It is placed conservatively, because the perioral area shows overfilling quickly.
For lines bound up with surface texture and sun damage, fractional resurfacing may be considered in selected patients to improve skin quality alongside, or instead of, injectables. Whether it fits depends on your skin type, pigment risk, and how much downtime you can take.
This is why assessment matters: many patients do best with a combination, for example a light Botox dose to calm movement plus a touch of filler for the etched lines, staged over time rather than everything at once.

Who May Be a Candidate
Good candidates are generally healthy adults bothered by vertical lip lines, with realistic expectations about softening rather than erasing them. If you currently smoke, lines tend to recur and results are less durable, Dr. Munib will be candid about that at consultation. Treatment is deferred during pregnancy or breastfeeding, with active infection or cold sores in the area, and with certain neuromuscular conditions for the Botox component.
Why Assessment Comes First
Because lip lines can come from movement, volume loss, skin texture, or a mix, treating the wrong driver wastes time and money and can look unnatural. The consultation is where Dr. Munib decides whether your lines are dynamic or static, how much your lip can safely support, and whether one treatment or a staged combination is the honest answer.
What to Expect
Most perioral treatments are quick office visits. Topical numbing is offered, and injections around the lip generally take under 20 minutes; resurfacing, when used, is a separate visit with its own preparation.
Downtime is usually modest, mild swelling, redness, or, with filler, some tenderness or bruising that typically settles within a few days. Botox softening tends to come in over several days, while filler shows immediately and refines over a week or two. How long results last varies with the treatment used, your anatomy, and whether you smoke; Dr. Munib reviews expected longevity for your specific plan at the visit.
Related Treatments
Vertical lip lines rarely sit in isolation, and a few related options often come up at consultation. lip filler for a deflating upper lip, a lip flip to roll a little more lip into view, marionette lines filler and nasolabial fold filler for the folds that frame the lower face, and fractional CO2 resurfacing for sun-damaged perioral texture.
You can also see a real Restylane smoker’s-lines before & after from the practice to set expectations for what softening, not erasing, looks like.
Quick Answers
Straight answers to the questions patients ask most about treating vertical lip lines.
Are smoker’s lines only caused by smoking?
No. Despite the name, most vertical lip lines are not caused by smoking. They commonly develop from normal repeated lip movement, collagen and elastin loss with age, sun exposure, and naturally thin perioral skin. Smoking can accelerate and deepen them, but plenty of people who have never smoked develop the same lines.
Will treating my lip lines freeze my smile?
It should not. Around the mouth, Botox is deliberately micro-dosed to soften the pleating motion without disabling the muscles you use to speak, eat, and smile. Dr. Munib treats this area conservatively precisely because over-treatment here reads as unnatural; the aim is a softer, still-mobile mouth, and dosing depends on your muscle strength and goals.
Botox, filler, or resurfacing for vertical lip lines?
It depends on what is driving your lines, which is what the consultation determines. As a general guide: lines that appear mainly with movement may respond to a light Botox dose; static lines etched in at rest often need a small amount of HA filler; and lines tied to sun-damaged surface texture may call for resurfacing. Many patients do best with a combination, staged over time rather than all at once.

I've gotten Fotona laser and microneedling for acne scars and laser hair removal done here, and Dr. Munib is absolutely amazing! Dr. Munib always makes sure that I was comfortable before, during, and after the treatment, and she checks up regularly following treatment to see how I was doing. I've seen great results following my treatments and will definitely be coming back!

After several aesthetic nurses told me I was allergic to filler dissolver, Dr. Munib took a careful, evidence-based approach. She used ultrasound to locate my migrated lip filler, tested a small area first, and explained I was reactive—not allergic. She dissolved a small test area before safely treating my upper lip. Throughout the process she was gentle, thorough, and educational. It’s a long drive for me, but her knowledge and expertise make it absolutely worth it.

Thank GOD for Dr. Munib. She is the angel of dissolver!!! I had HA nodules and Sculptra nodules from previously going to Dermfx and I went to Dr. Munib for ultrasound guided dissolving. She is gentle, patient, knowledgeable, kind and funny. I feel like she is the last of the good ones in this business. If you are interested in natural looking aesthetic results that aren’t botched and don’t cause you years of facial pain go to her. I would try to see her as my primary doc if I wasn’t moving out of the area. I would travel to go see her though.
