Why Dr. Munib does not perform calf Botox at Spectrum

The calves are working muscles. They keep you upright, they push you off the ground, and they protect your joints. When the literature shows that injecting botulinum toxin into the gastrocnemius produces subtle balance deficits, neurogenic atrophy that persists for months after a single dose, and incomplete recovery with repeat injections, those findings carry real weight — they are not theoretical.

Set against that, the reported aesthetic change is modest — typically around 0.3 to 0.7 cm of leg-circumference reduction at three to six months — and it is temporary, which means the patient must repeat injections to maintain it, and each repeat appears to layer additional structural change on top of the last. In Dr. Munib's view, asking a patient to accept long-term changes to a load-bearing muscle for a small, temporary cosmetic result is not a trade we can defend.

Dr. Munib's position

Dr. Sabeen Munib, MD does not offer calf Botox at Spectrum Skin Clinic. The reason is the gap between what the treatment delivers and what it asks of the muscle: a leg-circumference change of roughly half a centimetre at three to six months, in exchange for documented dynamic-balance deficits, neurogenic atrophy that holds for many months after a single injection, fibrosis and fat replacement layered in by repeat dosing, and weakness that has been reported to persist for years after cessation in other treated muscles.

The compensatory thickening described by Li and colleagues (2024) is its own quiet problem — the gastrocnemius thins but the soleus and subcutaneous fat thicken, which may erode the visual change the patient came in for. And the FDA prescribing label for BOTOX Cosmetic still carries a boxed warning on distant spread of toxin effect, which applies regardless of injection site.

Putting that picture in front of a patient who came in for a cosmetic line change is not a decision we are willing to recommend, in selected cases or otherwise. We would rather lose the appointment than place a load-bearing muscle into a multi-year cycle of partial atrophy for a small, temporary result.

The same label lists common local injection-site reactions and, for any large-muscle use, dosing and placement matter more — not less.

Calf injection for cosmetic slimming is an off-label use of BOTOX; it is not an FDA-indicated treatment area.
