Your face looks a little flatter or more tired than it used to, and three names keep coming up: Sculptra, Radiesse and PRFM. All three are described as ways to build collagen, so it is easy to assume they are interchangeable. They are not. Each one works differently, shows results on a different timeline, and suits a different kind of change.
The short answer
- Sculptra adds very little on the day; it prompts your skin to make new collagen over the following months, and results can last up to two years.
- Radiesse corrects folds where it is placed and, over the following weeks, increases collagen around its microspheres.
- PRFM uses your own platelets to support skin quality and may help soften fine lines, without adding manufactured filler material.
- Which one is better suited for you, depends on whether you have lost structure or skin quality, and on how long you are willing to wait for the result.
What each one is, in plain English
Think of the deeper layers of your face as scaffolding. With age, the skin loses collagen and the deep fat pads of the face shrink, which deepens folds such as the line from the nose to the mouth. Both Sculptra and Radiesse treatments support that scaffolding but the mechanism is different.
Sculptra is made of microparticles of poly-L-lactic acid, or PLLA. Once placed under the skin, the particles stimulate fibroblasts, the cells responsible for producing collagen. This process helps rebuild collagen gradually.
The PLLA itself is slowly absorbed. What you see in the mirror months later is the collagen your own skin made in response. Our Sculptra page covers the treatment itself in more detail.
Radiesse is made of tiny calcium hydroxylapatite microspheres suspended in a smooth gel. It corrects folds where it is placed. The gel is designed to provide immediate support in the treatment area. The microspheres also stimulate collagen and elastin which is why Radiesse is also considered a biostimulator.
PRFM, short for platelet-rich fibrin matrix, starts with a small blood draw at your visit. The blood is spun in a centrifuge, and the platelet-rich layer is mixed so that it sets into a soft fibrin gel within about ten minutes.
Once injected, that gel holds your platelets in place while they release growth factors over several days. Nothing manufactured goes into it; it is your own blood, prepared while you wait. You can read more on our Selphyl PRFM page.
PRF vs Sculptra: what actually differs
Patients often ask about PRF and Sculptra together because both are described as collagen builders. Sculptra and Radiesse are both biostimulators, products that prompt your own collagen, while PRF works through your platelets.
The real difference is purpose. Sculptra is a structural treatment, meant to restore support that has been lost across the cheeks and around the mouth and chin. PRFM is a skin-quality treatment, suited to fine texture, mild hollowing and skin that simply looks tired, and it does not rebuild deep support in the same way.
The timelines differ as well. Galderma, the company that makes Sculptra, describes a typical course of three sessions spaced at least three to four weeks apart. Patients can expect the results to show in about a month with results can last up to two years. These are general timeline, a guidance. The individual results may vary. PRFM may take one or multiple sessions with improvement developing over several weeks as the skin responds.
The right choice starts with what has changed in your face, not with the name on the box.
Patience matters with both. Neither treatment looks finished when you leave, and judging the result after a week usually leads to disappointment. With Sculptra in particular, the change is meant to arrive slowly, which is a large part of why it tends to look natural.
Where Radiesse fits, including Hyperdilute Radiesse
Radiesse sits between the other two. Because it corrects folds and also builds collagen, it suits people who want both. It is FDA-approved for moderate to severe folds, such as the lines from the nose to the corners of the mouth. It is also approved for restoring volume in the backs of the hands.
Radiesse can also be diluted so that it spreads thinly across a wider area. Used this way, the aim is firmer, better-quality skin rather than lift, and we offer it as Hyperdilute Radiesse.
Two practical points are worth knowing before you choose it. Radiesse is not used in the lips, because firm lumps have been reported there. Its microspheres also show up on X-rays and CT scans, so it is worth telling any doctor who orders imaging that you have had it.
Which fits which concern
| Your concern | What usually fits | What Dr. Munib weighs |
|---|---|---|
| Cheeks look flatter and the face looks tired | Sculptra, over several sessions | How much support is lost, and whether you can wait months for the result |
| Deep lines from the nose to the mouth | Radiesse or Sculptra | Whether you want fold correction plus collagen, or gradual collagen alone |
| Fine texture, mild hollowing, dull skin | PRFM | Whether the concern is skin quality rather than lost structure |
| You want nothing manufactured in your skin | PRFM | Whether your goal is realistic with your own blood alone |
What a treatment day is like
Each visit is short, and most people return to their normal plans the same day. This is the order things happen in.
- Assessment. Dr. Munib evaluates your face and confirms with you which treatment fits.
- Preparation. For PRFM, a small amount of blood is drawn and prepared while you wait; Sculptra is mixed ahead of time so it spreads evenly.
- Injection. Small amounts are placed through a fine needle into the planned areas, a little at a time.
- Aftercare. After Sculptra, the manufacturer advises massaging the area for five minutes, five times a day, for five days.
Mild redness, swelling, tenderness or bruising are common after any of the three, and they usually settle within a few days to two weeks. Many people plan their first session at least two weeks before an event for that reason.
Safety, and who performs the injections
All three are injections. The most serious risk the FDA describes for dermal fillers is product accidentally entering a blood vessel. It is rare, and it is the reason anatomy is assessed carefully before anything is injected. At Spectrum, every one of these injections is performed by Sabeen Munib, MD.
With Sculptra, small lumps can form under the skin. Spreading the product evenly and following the massage advice is how that chance is kept low. PRFM, made from your own blood, avoids reactions to manufactured materials, although bruising and swelling can still happen.
Sculptra has not been shown to be safe in pregnancy, while breastfeeding or under 18, and dermal fillers in general are approved for adults aged 22 and older. If any of that applies to you, we will talk about other options instead.
Just a real story to share here. If you are still buying on price first and safety second disregarding the fact as to who is your injector, this case shows how quickly a filler complication can become serious and why timely treatment matters.
A patient came to Spectrum after receiving hyaluronic acid filler elsewhere. A complication had restricted blood flow to part of the treated area, placing the tissue at risk of permanent damage. The patient noticed the change and sought help quickly. Dr. Munib dissolved the filler under ultrasound guidance. Blood flow improved, and the tissue recovered.
Early recognition and timely treatment mattered in this case. Your injector and your safety matter.
Who is a good candidate for a biostimulator or PRFM
These treatments suit people who want a natural-looking change and are happy to let it build.
- You notice gradual volume loss rather than one hollow spot
- You prefer a subtle change to an obvious, done look
- You can wait weeks to months for the full result
- You are not pregnant or breastfeeding
They suit you less if you want an instant result in a single area, or one that can be reversed. In that case a hyaluronic acid filler, which can be dissolved, is often the better first step; our dermal filler brands page explains the options. If your main concern is loose skin rather than lost volume, energy-based Skin Tightening may fit better.
How we assess it at consultation
The choice is made during an in-person consultation. Photos alone do not provide enough information to assess their face correctly. Dr. Munib looks at where support has been lost, the quality of your skin, any fillers you have had before, and how quickly you want to see change. She then recommends one treatment, a combination, or none at all.
More than one treatment may be considered when different areas of the face have different needs. These treatments can also be paired with other options as part of a non-surgical facelift. If you have concerns about filler placed elsewhere, we also offer a physician second-opinion evaluation.

When to seek urgent care
Contact us right away, or seek emergency care, if you notice any change in your vision after an injection. Do the same if skin turns pale, grey or blue, or if pain keeps getting worse instead of better. These can be signs that product has entered a blood vessel, and early treatment matters.
Frequently Asked Questions
What's better, PRP or Sculptra?
Neither is better in general, because they do different jobs. Sculptra is a collagen stimulator for lost volume, while PRP and PRFM are prepared from your own blood to improve lines and skin. Which one fits depends on what has changed in your face.
Are fillers or Sculptra better for me?
A hyaluronic acid filler shows its result the same day and can be dissolved if needed. Sculptra builds slowly over months and can last up to two years, but it cannot be dissolved. Fillers suit a quick, precise change in one area, while Sculptra suits gradual volume loss across the face.
What are the differences between PRP and PRF for facial treatments?
Both are made from your own blood. PRP stays liquid, while PRF, which we use as PRFM, sets into a soft fibrin gel that holds your platelets in place. That gel releases growth factors over several days instead of all at once.
Does PRF last longer than PRP?
Yes, it is designed to. Because PRF forms a gel that releases growth factors gradually, its effect is slower and steadier than liquid PRP. How long your result lasts still depends on your skin, your age and how many sessions you have.
Why don't some plastic surgeons like Sculptra?
Sculptra asks for patience and careful technique. Results take months, it cannot be dissolved, and lumps can form. That is why the product is spread evenly and the manufacturer's massage advice matters afterwards.
How long does Sculptra last?
Galderma, the manufacturer, reports results lasting up to two years. Most people see change from about a month, and it keeps building as new collagen forms.
