Midface Volume Loss, Sagging and Laxity, Collagen Loss, Hollow Cheeks

Patient's Age:
50's

Treatment:
Sculptra

Practitioner:
Sabeen Munib M.D.

Details of the Treatment:
She wanted gradual midface restoration — nothing visible to colleagues or family overnight. Cheek volume had reduced over years, and she had specifically researched collagen-stimulating options over hyaluronic acid filler.
Mid and lower-face assessment showed bilateral malar volume loss with mild associated lower-face descent. The skin elasticity was age-appropriate. The pattern reflected age-related midface volume change typical of the fifth decade. The plan was Sculptra at the deep malar position, with the dose calibrated for gradual collagen induction rather than immediate volume change.
Sculptra was reconstituted and placed via cannula at the deep malar position bilaterally. Poly-L-lactic acid is broken down by macrophages, with the resulting fibroblast response producing native collagen gradually. The technique requires patient counseling about gradual onset — patients expecting hyaluronic acid filler immediacy are often disappointed early on. In the after image, the midface volume has restored visibly, the lower face appears supported, and the collagen response continues to mature. The patient's spouse did not notice the change unprompted. Gradual restoration matches patients who want family and colleagues unaware of the intervention.
Individual results vary. The outcome shown reflects this patient's specific anatomy, health history, and treatment plan. Results shown are not guaranteed and are not necessarily typical. Photos used with patient consent.

Collagen Biostimulation, Not Filler
Sculptra (poly-L-lactic acid) stimulates the patient's own fibroblasts to produce new collagen over 3–6 months — genuine tissue regeneration rather than volume added by a foreign material.
Gradual, Natural-Looking Result
Volume improvement developed progressively across the midface, cheeks, and temples over 6 months — a change that reads as the patient's own tissue rather than introduced product.
Results Last 2–3 Years
Collagen stimulation from a full Sculptra series lasts 2–3 years — versus 9–18 months for HA filler — reducing the frequency of maintenance treatments required.
