Consultation: ultrasound imaging is used to map existing filler, including depth, location, and whether it has moved from where it was placed. This step is what separates a targeted correction from an estimate.

Hyaluronidase: What It Dissolves, and What It Does Not
Hyaluronidase is an enzyme that breaks down hyaluronic acid. Because most modern dermal fillers are hyaluronic acid based, those products may be dissolvable when correction is appropriate. Spectrum uses brand-name Hylenex recombinant for filler dissolution when hyaluronidase is appropriate, placed into deposits identified on imaging. The enzyme acts on the filler it reaches, which is the reason placement matters more than quantity.
Not every filler responds. Biostimulators such as Sculptra and Radiesse work by a different mechanism and are not reversed with hyaluronidase. Permanent and semi-permanent products are not dissolvable either. Where the injected product is unknown, or where records from a previous provider are unavailable, imaging and history are used to narrow down what is likely present before anything is offered.
Hyaluronidase is a prescription medication, administered by a physician. Using it to dissolve dermal filler is an off-label aesthetic use. It is well established in aesthetic practice, but it sits outside the product's approved indications, and that is worth stating plainly. Prior reaction to hyaluronidase is reviewed before treatment, and it is not appropriate for every patient. The degree of improvement depends on the filler type, its depth, how long it has been in the tissue, and how that tissue responds. Some findings improve substantially. Others improve partially, and a small number are better managed by leaving the filler to resorb on its own.
Native hyaluronic acid in the skin can also be affected, which is why treatment is directed at mapped deposits rather than spread across an area. This is the reasoning behind a staged, conservative approach: additional treatment can be considered at a second visit if needed, but tissue that has been over-treated cannot be put back.

Thank GOD for Dr. Munib. She is the angel of dissolver!!! I had HA nodules and Sculptra nodules from previously going to Dermfx and I went to Dr. Munib for ultrasound guided dissolving. She is gentle, patient, knowledgeable, kind and funny. I feel like she is the last of the good ones in this business. If you are interested in natural looking aesthetic results that aren’t botched and don’t cause you years of facial pain go to her. I would try to see her as my primary doc if I wasn’t moving out of the area. I would travel to go see her though.
