Chin and Nasolabial Fold Filler Dissolve Before & After | Lower Face Reset

Filler dissolve was the right step, not more product. Filler had built up at both the chin and the nasolabial folds — treated separately, in different chairs, over years — and the lower face had grown heavy.

On assessment, the lower face showed built-up product at the chin and along the nasolabial folds with secondary heaviness across the lower-face contour. The previous placements did not sit cleanly along the natural anatomic lines. The marionette area had also developed mild secondary fullness. The pattern reflected long-term layered filler across multiple zones without anything cleared in between. Hyaluronidase suited this presentation because resetting the lower face to baseline is the only way to evaluate what — if anything — actually needs new product.

Hyaluronidase was placed in small, paced amounts at the chin and along the nasolabial folds to clear the built-up product. Multi-zone dissolve calls for this kind of pacing because clearing large areas in one aggressive pass produces dramatic short-term changes that the patient finds disconcerting. Smaller, paced placements let the lower face settle as the product clears. In the after image, the chin reads natural in profile, the nasolabial folds have softened, and the lower-face heaviness has resolved. The face returns to baseline before any new product is considered.
