Overview
Selphyl PRFM Injections in Irvine
Selphyl PRFM is made from your own blood. A small amount is drawn and spun in a centrifuge, and the platelet-rich portion is set into a soft gel. Dr. Sabeen Munib, MD, then injects it into areas of the face, including the under-eyes where that suits you.
PRFM stands for platelet-rich fibrin matrix. Fibrin is a protein in your blood. In PRFM it forms a soft mesh that holds the platelets in place instead of leaving them in a liquid.
Injection is not the same as microneedling. If you want PRP or PRFM worked into the skin through microneedling, that is a separate treatment: PRP/PRFM microneedling.

One Physician Performs Every Injection
Selphyl PRFM is made at your appointment, from your own blood, minutes before it is used. There is no manufactured vial with a label on it. What is injected is what that day's preparation produced.
Injectables at Spectrum are performed by Dr. Sabeen Munib, MD. That includes every Selphyl PRFM injection.
No part of the injection is handed to an esthetician.
The person who decides whether PRFM suits you is the same person who injects it. When filler, a laser treatment or nothing at all is the better answer, she will say so.

What Platelet Treatment Is Realistically For
Under-eye Selphyl PRFM injection by Dr. Sabeen Munib, MD at Spectrum Skin Clinic, Irvine.
Look at the texture and the shadow under the eye rather than for added volume. This patient's darkness came from pigment and uneven surface, not hollowness, so PRFM was chosen as a skin-quality treatment. The change shows in how evenly the skin reflects light.

How the treatment runs
What Happens in a Session
The same four steps sit behind every Selphyl PRFM injection here.
Assessment
Establish whether a platelet preparation is the right answer at all, or whether something else is
Blood draw
Start and finish the process in one visit, with nothing stored or carried between rooms
Preparation
Prepare PRFM from your own blood at the same visit
Injection
Place the material where the problem actually is
60 to 90 minutes
Most of that is assessment and preparation rather than treatment time
PRP and PRFM Are Not the Same Preparation
Conventional PRP is collected into a tube containing an anticoagulant and centrifuged. What comes back is a platelet concentrate that stays liquid, and most of its growth factors are released in the first day.
Selphyl PRFM starts the same way. Blood is drawn into a tube containing sodium citrate, which keeps it from clotting. The platelet-rich portion is then moved to a second tube containing calcium chloride. That lets fibrin form, and the material sets into a soft matrix over about ten minutes.
Why the fibrin matrix matters
The fibrin is a scaffold. It holds the platelets in place rather than letting the whole payload disperse at once, which is the mechanical difference between the two preparations.
The matrix is thought to release growth factors more slowly than liquid PRP. Whether that gives a better result in any one person is not settled, and we do not claim it does.
Selphyl is not anticoagulant free. Any description saying otherwise is describing a different system.
Choosing between them
Injection or Microneedling?
Where the material goes, and how it gets there, matters more than the preparation.
Combined approaches
What It Is Sequenced With
Platelet treatment is more often one part of a plan than the whole of it.
Candidacy
Who May Be a Candidate
Selphyl PRFM injections may suit people who want to work on skin quality with their own blood rather than filler or a neuromodulator. Dr. Munib decides whether it suits you at an in-person consultation.
It is not right for everyone. Active infection in the area, certain blood disorders, current anticoagulation and some medications make it unsuitable. Pregnancy and breastfeeding are handled case by case.
How Dr. Munib decides
What you want changed, whether the problem is skin quality or lost volume, your medical history, current medication and prior treatment. Whether PRFM is injected at all, and where, follows from that rather than from a menu.

Likely a good fit
Skin quality concerns such as texture, early lines and thin under-eye skin, and a preference for your own blood over filler or a neuromodulator.
Where it is limited
It does not add volume, so a true hollow or deep fold is a filler conversation. Active infection in the area, some blood disorders and current anticoagulation mean deferring or declining. Results vary between people and are not permanent.
What happens
What to Expect
Injection, particularly under the eye, can bruise and swell. Plan around anything you need to look settled for.
Risks
Risks and What to Expect
Autologous material lowers some risks and removes none of the ones that come from technique and handling.
Bruising and swelling
Common with injection, particularly under the eye. It usually settles on its own.
Redness
Injection points may look red for a short time.
Infection
Uncommon, and the reason preparation and handling happen in a clinical setting rather than a spa.
Results that fall short
Some patients see less change than they hoped. Where that is likely, it is better said before treatment than after.
Regulatory status
The Selphyl system is listed with the FDA as a device for preparing platelet-rich plasma from your own blood. It is not approved or cleared for injection into the face. Facial use is off-label and directed by the physician.
What the treatment can and cannot do is covered before the first session, in writing.
Why a physician does this
Preparing and handling material made from your own blood is where avoidable problems come from. Dr. Sabeen Munib, MD, performs every injection.
Off-label means the device is used in a way its FDA listing does not cover. That is lawful physician practice. It is not approval for facial use, and we do not describe it as one.

What Affects Your Cost
We quote Selphyl PRFM injections at consultation, after Dr. Munib has assessed the areas you want treated.
New patients receive 15% off their first treatment.
The amount of material and the number of sessions vary with the area, so a series is discussed rather than sold in advance. PRP/PRFM microneedling is a separate treatment with its own price.
New patients: 15% off your first treatment.
What to expect
What Change to Expect
Any change is gradual, because it depends on how your own skin responds. Some people see less change than they hoped, and if that seems likely, Dr. Munib will say so before treatment.
It is not permanent. Skin keeps aging, and whether further sessions make sense is decided once Dr. Munib has seen how yours responds.

No. Here, PRFM is injected into chosen areas of the face. PRP/PRFM microneedling works the material into the skin through tiny channels made with a needling pen, and it is a separate treatment. Dr. Munib can help you decide which suits your skin at consultation.
