Details of the Treatment:

Rosacea had been worsening for two decades — persistent redness, visible cheek vessels, flushing on mild triggers. Topical metronidazole and oral doxycycline had calmed the inflammation but not the visible redness.

Cheek and midface assessment showed diffuse erythema with scattered telangiectasias bilaterally, characteristic of long-standing erythematotelangiectatic rosacea. The skin was Fitzpatrick type II. There was no active inflammatory papulopustular activity. The pattern reflected the vascular subtype of rosacea predominating over the inflammatory subtype. The plan was IPL with the medical management for the inflammatory component continuing in parallel.

IPL was performed across both cheeks and the central face at vascular-targeting wavelengths. IPL produces selective photothermolysis of vascular targets — the abnormal vessels absorb the light and coagulate, while surrounding tissue is unaffected. Recovery is approximately one to two days of mild redness. In the after image, improvements of this kind are commonly observed across a course of treatment, the diffuse facial erythema has substantially reduced, the telangiectasias have cleared, and the resting facial color reads as normal. Vascular IPL addresses the cosmetic component of rosacea that medical management cannot. The two interventions complement rather than replace each other.
