Fractional CO2 was the path here because both wrinkle formation and early jowling were showing up at once. Surgery was off the table for timing reasons.

Full-face assessment showed fine to moderate rhytids around the eyes and mouth, mild surface texture irregularity, and early skin laxity at the lower cheeks and along the jawline. The skin was photoaged but free of significant solar lentigines. The pattern reflected combined dermal collagen attenuation and early elastotic change. The plan was full-face fractional CO2 at moderate-high density, with the lower face and jawline receiving the same density as the upper face to address the laxity component.

Fractional CO2 was performed across the full face at moderate-high density. The lower face received an additional pass to maximize collagen induction in the laxity zones. Fractional CO2 induces dermal heating that triggers collagen remodeling — the laxity improvement appears gradually rather than immediately. Recovery involves a healing period followed by pink tone that resolves. In the after image, results of this kind are typically observed over several months as collagen rebuilds, the rhytids have softened, the surface texture has smoothed, and the lower-face contour shows visible tightening. Fractional CO2 has limits — surgical-grade laxity cannot be addressed with energy-based platforms. For the right patient at the right stage, the limit is high enough to delay surgery by years.
