Nasolabial Fold Filler Before & After | Smoother Midface Transition

Smile lines from nose to mouth had shown up earlier than typical for her — and they were her only complaint. No cheek volume loss, no other lower-face changes.

On assessment, both sides showed defined smile lines from nose to mouth with healthy cheek volume holding above. There were no other midface or lower-face changes. The skin was elastic. The pattern reflected natural anatomy rather than age-related change. A direct approach suited this presentation because adding cheek filler in a young patient with healthy cheek volume would have produced overcorrection.

A soft filler was placed by cannula along both smile lines at conservative volume. Direct line treatment in young patients with isolated line depth is more disciplined than the bundled midface approach common in older patients. The patient's actual complaint determines the placement. In the after image, the smile lines have softened, the surrounding face is unchanged, and the natural cheek-to-mouth transition reads smoother. When one zone is the complaint, one zone is the treatment.
