On assessment, both cheeks showed reduced volume, the smile lines from nose to mouth had deepened, and the lower face had begun a gentle descent. The chin position was holding. The skin was age-appropriate in elasticity. The pattern reflected combined age-related midface change typical of this decade. A staged single-visit approach suited this presentation because much of the smile-line depth resolves on its own once the cheek volume is restored.

A firmer filler was placed at the deep cheek position on both sides to restore the midface volume and bring the cheekbone forward. A softer filler was added in the smile lines at conservative volume. The chin and jawline were left alone — neither was contributing to the front-view complaint. Treating the smile lines as a downstream effect of midface volume loss rather than a separate target is the framework. In the after image, the front-view midface reads restored, the smile lines have softened, and the lower face appears supported.
