Lip Filler Swelling Stages: A Day-by-Day Guide

What lip filler swelling looks like on day one, days two and three, week one and week two, what helps it settle, and when to call us.

You just had lip filler, and now your lips look bigger, firmer or a little uneven, and you keep checking the mirror. Is this normal, and when will it settle? Swelling after hyaluronic acid lip filler follows a fairly predictable pattern. This guide walks through each stage, what helps it calm down, and the few signs that need a call right away.

The short answer

  1. Some swelling right after lip filler is normal, and it starts very shortly after the injections.
  2. Bruising may be immediate or get darker over the first 3 days.
  3. Most injection swelling settles within 1 week, and common side effects usually last 2 weeks or less.
  4. White or dusky skin, severe or unusual pain, or any change in vision needs a call right away.

Why lips swell after filler

Some transient swelling right after treatment is normal and occurs with all dermal fillers. It starts very shortly after the injection, and how much you get relates to the injection volume and technique. The filler itself is hyaluronic acid (HA), a naturally occurring sugar found in the body, used to temporarily plump the lips.

Swelling is rarely the only thing you notice. The most common side effects are temporary responses at the treatment site such as swelling, tenderness, bruising, firmness, and lumps or bumps. That is why lips can look lumpy or uneven in the first days, and why that early look is not your result.

Signs your healing is on track

Normal healing after lip filler usually looks like this:

  • Lips look fuller right away, even before swelling settles.
  • Swelling starts soon after the injections, then eases within a week.
  • Tenderness, firmness or small bumps that usually fade within 2 weeks.
  • Bruising that shows early and fades over days to weeks.
  • Normal skin color, with no white, gray or dusky patches.

How to reduce lip filler swelling

These steps follow the aftercare in the filler manufacturer's patient labeling.

  1. Use a cold pack. If there is swelling, place an ice pack over the swollen area for brief periods.
  2. Skip the workout. Minimize strenuous exercise for the first 24 hours, because it may increase redness, swelling and itching.
  3. Avoid sun and heat. Keep out of strong sun, heat and tanning beds in the first 24 hours, since they may increase swelling.
  4. Keep your hands off. Avoid touching the treated area for three days unless we tell you otherwise.
  5. Check your medicines. Tell us if you take aspirin, ibuprofen or other blood thinners, which may increase bruising.

Who should wait or skip lip filler

Lip filler is not for everyone. Dermal fillers, including lip fillers, are approved for adults over the age of 21. People with a history of severe allergic reactions (anaphylaxis), or an allergy to lidocaine, should not have a lip filler that contains lidocaine.

Tell us if you are pregnant or breastfeeding, because safety in both has not been studied. Tell us about cold sores too, because filler injections can reactivate the herpes virus and preventive medicine before treatment may lower the chance. Tell us if a dental visit, vaccine or laser treatment is coming up, since inflammation near filler has been reported after these.

Results, touch-ups and how long lip filler lasts

Common side effects usually last 2 weeks or less. Most fillers offer temporary results and need repeat treatments to maintain them.

Judge your lips at two weeks, not two days; early swelling hides the real shape.

When to seek urgent care

Rarely, filler can block a blood vessel and cut off blood supply to the tissue, which can be serious and may be permanent, including tissue death, vision loss or stroke. Call us right away if you notice white or blanched skin, a lacy purple pattern, dusky blue-red discoloration, or pain that is severe or unusual. Call 911 for any change in vision or signs of stroke, such as sudden trouble speaking, face drooping or weakness in an arm or leg. Early treatment with hyaluronidase, an enzyme that breaks down HA, is a mainstay of care.

Frequently Asked Questions

How long does lip filler swelling last?

Most swelling from the injections dissipates within 1 week. Common side effects such as swelling, tenderness and lumps usually last 2 weeks or less, and most go away on their own within 30 days.

When is lip filler swelling the worst?

Swelling starts very shortly after the injections, so the first days are usually the fullest. Bruising can lag behind and may get darker over the first 3 days before it fades.

Can lip filler look uneven at first?

Yes. Swelling, firmness and small lumps or bumps are common early reactions that usually last 2 weeks or less.

Will Benadryl help lip filler swelling?

Ask us before taking any antihistamine or other medicine after filler. Antihistamines are one of several treatments doctors may prescribe for some filler side effects, and the right choice depends on what is causing the swelling.

Does lip filler look better after 2 weeks?

For many people, yes. Common side effects usually last 2 weeks or less. Tell us if anything still looks uneven at that point.

Can lip filler move after it settles?

Migration, meaning filler moving away from where it was placed, is a rare reported risk. Our guide to filler migration explains the signs and how it is corrected.
