Tear Trough Filler Dissolve and Reset Before & After | Periorbital Correction

Her instinct was more filler to balance the look. She stopped herself when she realized the issue was placement, not volume — the original tear trough work had drifted into an undefined fullness that no longer matched her face.

On assessment, the under-eye area showed product that no longer sat where it had originally been placed. There was mild puffiness at the inner corner where filler had drifted. The lower-eyelid skin itself was healthy. The pattern was normal long-term filler movement in a high-mobility part of the face. A reset approach suited this presentation because the only way to plan the next step was to see the patient's own anatomy first.

Hyaluronidase was placed in small, paced amounts across the tear trough area. After the dissolve had settled, the patient elected not to pursue new filler. The under-eye hollowing she had originally treated had not returned to its previous depth. In the after image, the under-eye anatomy reads as her own. The face returns to baseline before any new product is considered. Sometimes the right new placement is no placement at all.
