She wanted lip augmentation that didn't look augmented — lips that read plausibly like her own at a fuller stage of life. The fear was the same one most first-time patients describe: overdone results visible in photographs.
On assessment, the lips showed reduced volume in both upper and lower lip, with a soft lip border and a flattened cupid's bow. The lip-to-face proportions were within range. The patient was healthy. The pattern was natural thinness rather than acquired volume loss. A conservative approach suited this presentation because the goal was a result that read as the patient's own lips, only a little fuller.
A soft filler was placed by cannula in conservative amounts across both lips, with the lip border addressed first. Total product was on the lower end of what the lips could carry. The lower end of the volume range tends to produce the most natural-looking single-visit results. In the after image, the lip border is defined, the body volume has modestly increased, and the cupid's bow has emerged. The lips look like a more rested version of the patient's own. Less filler placed correctly outperforms more filler placed for visible effect.
