She wasn't seeking the lips of her 30s. She wanted the lips she had ten years ago — back to where they were before the thinning showed in photographs.

On assessment, both lips showed substantial reduction in body volume, with a softened lip border and visible fine lines around the mouth. The cupid's bow was flattened and the columns between the nose and the upper lip were less defined. The pattern reflected progressive age-related volume loss, accelerated through the post-menopausal window. The lip border had pulled back on both sides. A conservative restoration approach suited this presentation because the underlying anatomy was intact — the structures were simply thinned.

A soft filler was placed by cannula in two passes. The lip border was addressed first to re-establish the structural edge. Small amounts in the upper and lower lip body then restored the lost volume. The total volume was modest, matched to her face in her current decade rather than to a younger lip ideal. In the after image, the lip border is once again visible, the lip body has returned to a proportionate volume, and the smile reads as the patient's own. Restoration over augmentation is the framework, particularly in patients who have watched their face change and want it back, not modified.
