She wanted one conservative session and a chance to see how it settled before deciding on more. Naturally thin lips had been a source of self-consciousness since high school, but other providers had told her she'd need a starter plus follow-up syringes in a row.

On assessment, both lips were naturally thin, with a soft lip border and a flattened cupid's bow. The lip-to-face proportions were normal. The pattern was natural lip thinness rather than volume that had been lost. A conservative approach suited this presentation because the most common cause of unnatural-looking lips is volume that goes beyond what the patient's face can carry.

A soft filler was placed by cannula in conservative amounts across both lips, with the lip border addressed first. Total volume was on the lighter side. The goal was a visible but proportionate result in one session rather than building up over several visits. Conservative single-session treatment is more demanding because the placement has to be precise. In the after image, the lip border is defined, the body volume has modestly increased, and the upper-to-lower lip ratio is preserved. The patient declined any follow-up addition. What was placed was sufficient. Less filler placed correctly outperforms more filler placed for visible effect.
