The phrase she used was 'the same lips, just a little more.' Naturally thin and always had been — no age-related change, just the moment she felt comfortable considering filler.
On assessment, both lips were naturally thin with reduced volume in both the upper and lower lip but in proportionate distribution. The upper-to-lower lip ratio was within range. The lip border was soft. The cupid's bow was present but undefined. The pattern was natural lip thinness rather than acquired volume loss. A balanced approach suited this presentation because the lips already sat in proportion to one another.
A soft filler was placed by cannula in matching amounts on both lips. The lip border was addressed first. Body volume was added in equal portions above and below. Equal-distribution placement preserves the upper-to-lower ratio that gives a lip its natural look — disproportionate placement is the most common cause of the artificial look patients fear. In the after image, the lips have proportionate volume increase, the cupid's bow is more defined, and the upper-to-lower ratio is preserved. The lips read as a fuller version of the patient's own anatomy rather than a different lip.
