On close-up assessment, the lips showed combined thinness and surface change. The lip body volume was reduced. Fine vertical lines extended up from the upper lip border. The lip surface read dry, with reduced light reflection. The lip border was soft. The cupid's bow was flat. The pattern combined natural lip thinness with age-related surface change. A multi-zone approach suited this presentation because each concern called for a different placement strategy.
A soft filler was placed by cannula in four placement strategies in one visit. The lip border was defined first. Body volume was added in conservative amounts. Tiny shallow droplets across the lip surface improved hydration. Small amounts above the upper lip border softened vertical lines. Multi-zone placement in a single visit is technically demanding but more efficient than serial single-issue treatments. In the after image, the lip border is defined, the body volume has increased modestly, the lip surface reflects light evenly, and the vertical lines have softened.
