The lips no longer felt like hers after several rounds of filler at other clinics. The issue wasn't size — the shape had drifted from what she'd originally hoped for.
On assessment, the lips had a shape that no longer reflected the patient's natural anatomy. The lip border had drifted subtly. The upper lip projection was modestly excessive. The lower lip looked disproportionately full relative to the upper. The overall effect was a lip that read as filled but undefined. A staged approach suited this presentation: clear the existing distribution first, then rebuild the shape from baseline rather than layering more product on top of the previous result.
The previously placed filler was cleared first. The lip was then rebuilt with a soft filler in a planned distribution — lip border first, upper lip body second, and lower lip body restrained. Full-redesign cases work because patient and provider make the second decision with the lip at baseline rather than with previous product still in place. In the after image, the lip shape reflects the patient's anatomy with appropriate proportional volume — defined border, balanced upper-lower ratio, restored cupid's bow. The face returns to baseline before any new product is considered. This is the correct sequence when the previous result is the reason the patient is in the chair.
