Details of the Treatment:

Congenital lip asymmetry had become more visible as she aged — upper-lower ratio off, sides not the same, an uneven lip from any angle. She had been told elsewhere she needed a full lip overhaul, and was looking for something more measured.

Front-view assessment showed bilateral upper-lip asymmetry with the right side modestly fuller than the left, mild lower-lip asymmetry in the opposite direction, and an upper-lower ratio that read as bottom-heavy. The vermilion border was preserved. The cupid's bow was off-center toward the fuller side. The pattern was consistent with congenital anatomical variation rather than acquired asymmetry from prior intervention. The plan was a multi-zone unilateral approach. Left upper lip filler matched to the right. Right lower lip filler matched to the left. Small upper-lip border placement balanced the overall ratio.

A soft hyaluronic acid filler was placed by cannula in three zones in a single session. Left upper lip was filled to match the right side. Right lower lip was filled to match the left side. Upper vermilion border placement brought the upper-lower ratio into proportion. Asymmetric placement in three zones is more technically demanding than uniform bilateral filling. The goal is to make each zone disappear into the existing anatomy on the opposite side, not to add visible volume. In the after image, the upper and lower lips read as symmetric front-to-front, the cupid's bow has centered, and the upper-lower ratio is proportionate. What is already there determines what can be added. In asymmetric cases, the larger existing side dictates the smaller side's target volume.
