Under Eye Filler Before & After | Hollow Correction in a Younger Patient

Tear trough hollowing had been a lifelong feature for her — anatomic, not age-related. Concealer never covered it.

Tear trough assessment showed bilateral congenital hollowing along the medial orbital rim with secondary shadowed appearance. The lower-eyelid skin was healthy with full elasticity. There was no puffiness, laxity, or pigmentary contribution to the dark appearance. The pattern reflected anatomic morphology rather than acquired aging. The plan was conservative under-eye filler via cannula at deep periosteal level, with the total dose calibrated below the case standard to allow gradual rather than complete correction.

A soft hyaluronic acid filler was placed via cannula at the deep periosteal level. Conservative dosing in younger patients with congenital hollowing prevents the over-correction that comes with full-volume protocols. The result is subtler than fully corrected cases but more durable and easier to refine if needed. In the after image, the tear trough hollowing has improved, the dark shadow has lightened, and the lower-eyelid contour reads as natural. Conservative initial volume in a young patient is the framework. Augmentation can always be added; over-correction is harder to undo.
