Front-view tear trough assessment showed mild bilateral hollowing along the medial inferior orbital rim with mild secondary darkness from the shadow. The lower-eyelid skin was preserved. There was no significant puffiness or laxity component. The pattern was characteristic of early age-related orbital fat redistribution. The plan was conservative under-eye filler placed via cannula at deep periosteal level, with the total dose targeted at three-quarters of standard volume to match the patient's subtle-result constraint.

A soft hyaluronic acid filler was placed via cannula at the deep periosteal level in conservative volume. Cannula technique avoids the bruising and Tyndall risk of needle injection. Subtle results require restraint at the time of injection — the temptation to add more is what produces the obvious results patients want to avoid. In the after image at the eight-week mark, the tear trough has lifted, the under-eye shadow has reduced, and the periorbital appearance reads as more rested. The patient's family did not notice the change unprompted. Subtle is harder than obvious. The right placement and the right dose are not always the same as the maximum placement and the standard dose.
