Under Eye Filler Dissolve Before & After | Tear Trough Reset in a Mature Patient

A decade of layered under-eye filler had reached the point where more product was no longer the answer. The original tear trough hollowing had been replaced by a heavy, undefined fullness — and her last provider had still recommended adding more.

On assessment, the under-eye area showed widespread filler buildup across both lower eyelids with product that had drifted above the bony rim. The placement no longer matched the natural anatomy of the eye area. The patient's actual baseline was hidden under years of product. The pattern reflected long-term repeat injection without anything cleared in between. A staged dissolve approach suited this presentation because the priority was getting the patient's own anatomy back into view before any new decision could be made.

Hyaluronidase was placed in small, paced amounts across the entire under-eye area. Mature under-eye cases call for this kind of pacing because the tissue does not tolerate clearing large areas in one aggressive pass. The patient's actual anatomy emerged as the product cleared. In the after image, the lower eyelids read as her own — for the first time in years. The face returns to baseline before any new product is considered. In long-history cases, the original anatomy may not be what the patient remembers, and the dissolved-baseline image is often where the conversation stops.
