Botox Before & After | Forehead and Frown Line Smoothing in a Male Patient

Deep horizontal forehead lines and a sustained line between the brows had become his resting expression — colleagues were asking if he was upset when he wasn't. Results he had seen on other men had read as frozen.

On assessment, deep horizontal lines ran across the entire forehead with depth holding at rest. The lines between the brows were vertical and prominent — the result of decades of sustained activity. The brow position was within range for a male in this decade. The pattern reflected cumulative movement-driven change in a thicker male muscle layer than in female anatomy. A male-tuned dosing approach suited this presentation because the muscle is denser.

Botox was placed at six points across the forehead and five points between the brows. The dose was matched to male muscle mass — typically larger than the equivalent female dose. Placement preserved meaningful forehead movement to maintain natural eyebrow expression. Over-relaxation in male patients produces the frozen look that drives most male resistance to neuromodulators. In the after image, the horizontal forehead lines have softened and the line between the brows has flattened. The brow still moves on animation. Treating men is a different protocol than treating women — same product, different anatomy, different dose.
