On assessment, both sides showed flaring of the nostrils on smile and the nasal tip pulled downward on speech. At rest, the nose looked unchanged. The complaint was movement-only, not at rest. The pattern was the kind of dynamic muscle activity that often shows up with age in the muscles at the base of the nose. A small-volume approach suited this presentation because the muscles at the base of the nose are small and the effect spreads over short distances.

Botox was placed in tiny amounts at the base of the nose, both at the center column and on each side at the base of the nostrils. Doses were small — the muscles here are small and the effect spreads in short distances. In the after image, the flaring has reduced, the tip is no longer dropping on speech, and the dynamic profile reads more balanced. The nose at rest is unchanged because the nose at rest was never the complaint. Treat the function when the issue shows only on movement.
