On assessment, the chin sat slightly back from where it should have been. The lines from the corners of the mouth down toward the jaw had deepened and were visible at rest. The jawline was holding. The pattern reflected combined age-related changes typical of this decade. A staged approach suited this presentation because the chin position changes what the surrounding lines actually need.

A firmer filler was placed at the chin point first to project it forward into a natural position. A softer filler was then placed in the lines from the corners of the mouth to soften them. Lower-face balancing requires order — the chin position determines what the surrounding lines need, not the other way around. In the after image, the chin point reads forward, the mouth-corner lines have softened, and the lower-face contour has improved definition. The patient's chin position dictates the planning of the surrounding zones.
