The goal was restoration to her own baseline, not augmentation. Her lips had thinned visibly compared to where they used to be — earlier than typical age-related change but consistent with the weight loss she described.
Front-view assessment showed reduced lip body volume with the upper lip more affected than the lower. The vermilion border was attenuated. The cupid's bow was less defined than the patient described it being at her own baseline. The pattern was consistent with acquired volume loss in a young patient — likely soft-tissue redistribution rather than structural lip change. The plan was conservative bilateral restoration, weighted to the upper lip, with the goal of returning to the patient's documented prior volume rather than augmenting beyond it.
A soft hyaluronic acid filler was placed by cannula in two passes, with concentration on the upper lip and the vermilion border. Conservative restoration in young patients targets the documented prior baseline. Old photographs are useful planning references when the goal is recovering recent volume rather than building new volume. In the after image, the upper lip volume has restored, the cupid's bow is visible at rest, and the lip border reads as a defined line. The face proportions match the patient's prior photographs. Restoration over augmentation is the framework, particularly in young patients whose own anatomy is the only relevant reference point.
