PRFM Under Eye Before & After | Natural Volume and Skin Quality in a Mature Patient

Filler was off the table — Tyndall, migration, and the difficulty of correction had ruled it out in mature periorbital skin. The under-eye showed combined hollowing, mild puffiness, and crepey surface texture.

Mature periorbital assessment showed mild bilateral tear trough hollowing with overlying mild lower-eyelid puffiness, crepey skin texture, and reduced surface light reflection. The skin was Fitzpatrick type II. The pattern reflected age-related orbital fat redistribution combined with collagen attenuation in the seventh decade of life. The plan was PRFM injected superficially across the lower eyelid, with realistic expectations that the result would be subtle compared to filler.

PRFM was prepared from the patient's own blood draw and injected in superficial micro-aliquots across the lower eyelid. The injection depth was conservative for mature periorbital skin. PRFM produces cumulative collagen induction gradually rather than immediately. The result is subtler than filler but avoids the migration and Tyndall risks that filler carries in this region. In the after image, the surface texture has refined, the skin quality has improved, and the periorbital region reads as more rested. The volume change is minimal. PRFM has limits in mature patients. For the right patient, working within those limits is preferable to crossing them with filler.
