A raised keloid scar wasn't responding to topical care. Intralesional injection was the next step.
The lesion showed a firm, raised contour with a wider footprint than the original wound. Color was deeper than the surrounding skin. The pattern was consistent with a keloid response rather than a hypertrophic scar that would resolve on its own. The plan was intralesional steroid injection, with progress measured by lesion height and color rather than by complete resolution.
Intralesional injections were placed precisely into the lesion using a fine needle. Volume was titrated to lesion size, and the pattern was distributed across the keloid bed rather than concentrated at one point. In the after image, the lesion height has reduced, the color has lightened, and the surrounding skin is flatter. Keloid biology is patient. Steady, distributed treatment outperforms an aggressive single attempt.
