Details of the Treatment
She wanted both a slimmer lower face and relief from the nighttime grinding her dentist had been documenting on her molars. Mouth guards had reduced the dental damage but hadn't changed the jaw width.
On assessment with the patient clenching, the jaw muscles were visibly bulky on both sides. The lower face read wider at the jaw than at the cheekbones, giving a bottom-heavy shape. The wear pattern her dentist had documented was consistent with sustained nighttime grinding. The pattern was both a cosmetic and a functional jaw-muscle issue. Masseter Botox suited this presentation because it reduces both the muscle bulk and the grinding force without affecting her ability to chew normally.
Botox was placed in the lower part of the masseter on both sides, kept well clear of the salivary gland and the muscles around the mouth. In the after image, the lower-face contour has narrowed, the jaw angle reads softer, and the patient reports less morning soreness. One placement strategy addresses both the cosmetic complaint and the functional one. The medical and the aesthetic are not always separate plans.
