TMJ pain was the entry point — chronic morning jaw pain, audible clicking on opening, tension headaches her doctor had attributed to stress. The visible jaw bulk was the secondary concern.
On examination, the masseter muscles were tender on both sides with audible clicking on opening. The jaw muscle bulk was visible at rest, more on the right than the left. The lower face read wider at the angle of the jaw than at the cheekbones. The pattern was sustained jaw overactivity producing both joint dysfunction and visible widening of the lower face. Masseter Botox suited this presentation because both the cosmetic and the functional concerns trace back to the same muscle.
Botox was placed in the masseter on both sides, with a slightly higher dose on the right to match the asymmetry. Placement avoided the salivary gland and stayed within the safe zone defined by feeling the muscle on clench. In the after image, the lower-face contour has narrowed, the jaw angle reads softer, and the patient's morning pain has resolved. The functional and the cosmetic are not always separate problems. In masseter cases, one placement strategy addresses both.
