She had reached the end of what concealer could do. Years of trying to fix the look with more filler had drifted the product, blurred the lip border, and made the upper lip heavy at rest — the lips didn't feel like hers anymore.

On assessment, drifted product and buildup were visible across the entire lip area — above the lip border on both sides, in the body of the upper lip, and along the lower lip toward the corners. Telling natural lip from filler was difficult on inspection alone — the late-stage look of compounded layered injection. The lip border had lost most of its definition. A full-reset approach suited this presentation because the lips needed to come back to baseline before anything new could even be considered.

Hyaluronidase was placed in small amounts across all visible product zones, paced across more than one visit to avoid the trauma of an aggressive single-pass dissolve. The lip body was approached separately from the skin above the lip border, since these contained different distributions of product placed at different times. Full-reset cases take time, not weeks, and the patient is shown the lip at baseline before any conversation about new filler begins. In the after image, the patient's actual lip anatomy is visible — for the first time in years, in her words. The face returns to baseline before any new product is considered. This patient declined to add anything new. The lip she had at the start was the lip she wanted back.
